In the 'St Helens People's Plan', health and social care partners have outlined their intention to work together as a partnership.
This new partnership agreed a new and ambitious shared vision of 'Improving the lives of people in St Helens together by tackling the challenge of cost and demand'.
To support the delivery of this ambitious vision, St Helens Cares, a local care management system was established.
St Helens Cares brings together all local service providers, who will become jointly responsible for the quality and cost of care for local people, working together within agreed budgets.
St Helens Cares will take a 'place based' working approach, breaking down organisational boundaries and working together as partners to meet the needs of people in St Helens.
As part of this, we are working closely together on a number of major transformational programmes which are overseen by the People's board. We are represented on the People's Board by John Heritage, Chief Operating Officer.
Organisations include:
- NHS St Helens Clinical Commissioning Group
- North West Boroughs Healthcare NHS Foundation Trust
- St Helens and Knowsley Teaching Hospitals NHS Trust
- Torus
- Healthwatch St Helens
- Halton and St Helens Voluntary Community Action
- St Helens Council
