Warrington Together means health and social care services working together, with local residents, to create a healthier, happier Warrington.

Warrington Together will focus on primary care - making sure local people have access to the right services at the right time, and prevention - supporting and empowering people to take ownership of their own health and wellbeing and stay independent for longer.

Partners will build on strong foundations to make better use of precious resources. Services will be brought together at convenient locations borough-wide, where residents can access the ongoing support they need to manage their own health and wellbeing. Those who need help the most can rely on the most support.

Local residents will play a key role in shaping the delivery of health and social care provision. Rising demand and limited resources mean we can't afford to carry on doing what we've always done - but what we do in the future depends on what we learn from listening carefully to local people, service users and our staff.  

Warrington Together will be owned by all of us, designed by all of us and delivered by all of us. This all adds up to a healthier, happier Warrington.

Warrington's health and social care partners will move a step closer to truly joined-up working with the creation of a multi-disciplinary team (MDT) for Central North Warrington from December. 

Through the Warrington Together programme, our Trust is working with local partners to deliver improved, integrated services to the people of Warrington. 

Central North is a 'test bed' for the new approach and involves people from our Trust, Bridgewater Community Healthcare NHS Foundation Trust and Warrington Borough Council. 

A multi-disciplinary team will be made up of district nurses, community matrons, social workers, Warrington Wellbeing and mental health professionals. They won't work under one roof at this stage, but they will work together to devise and deliver bespoke packages of citizen-centred care, enabling people to live safely and independently in their community. 

Central North MDT will be evaluated in spring 2019, with lessons learned being applied before further roll out to other parts of Warrington.
